Registered Nurse (RN) Case Manager Responsibilities

  • Oversee and manage an assigned nursing home household with a strong focus on person-centered care
  • Guide and support household staff in meeting the physical, emotional, and psychosocial needs of residents
  • Administer medications and treatments in accordance with physician orders and facility policies
  • Coordinate resident admissions, discharges, and transfers to and from the household
  • Complete the Resident Assessment Instrument (RAI), including MDS assessments, CAAs, triggers, and individualized care plans for assigned residents
  • Perform nursing skills requiring RN licensure, including IV therapy and blood draws
  • Process, transcribe, and implement physician orders in compliance with established policies and procedures
  • Maintain current knowledge of emergency protocols and respond effectively to emergent situations

Qualifications

Registered Nurse (RN) Case Manager

Qualifications

  • Current Registered Nurse (RN) license to practice in the state of Minnesota
  • Current Basic Life Support (BLS) certification, including CPR and AED
  • Minimum of two years of nursing experience; long-term care experience preferred
  • Demonstrated leadership ability to develop, motivate, and guide staff; strong critical-thinking and problem-solving skills; ability to prioritize responsibilities, remain flexible, and perform effectively in high-stress situations
  • Excellent verbal and written communication skills in English; ability to work independently, exercise sound judgment, and collaborate effectively as part of a team
  • Proven management skills, including planning and oversight of resources and the design and improvement of work processes
