Salary Range: Annualized salary range of $ 71,655.00 - $ 76,728.00, commensurate on skills and experience plus optional HOOPP participation
Position Reports to: Manager, Integrated Care and Experience - Black Health & Wellness
Primary Location: North Oshawa; mobile clinic sites (with flexibility to work/transition to any DCHC locations as required: Pickering & South Oshawa sites).
Who We Are: Durham Community Health Centre (DCHC) is a charitable registered organization that provides integrated, accessible, and equitable community-based primary care, wellness services, and health education for Durham community members facing barriers to health and well-being.
Who We Serve: DCHC offers equity-focused programs and services for priority populations, including Indigenous peoples, Black communities, 2SLGBTQI+ community members, newcomers to Canada, seniors, and clients without a family doctor. This approach helps community members access both preventive care and care when they are ill.
How We Work: Our interprofessional team includes physicians, nurse practitioners, nurses, counsellors, dietitians, outreach workers, medical secretaries, and administrative staff. Together, we place each client at the centre of care and respond to their needs through integrated clinical care, wellness support, and health education.

Working as a part of the, interprofessional care team, the Registered Nurse (RN) will deliver comprehensive, client-centered, and developmentally appropriate with a focus on mental health promotion, prevention, and culturally safe care for African, Caribbean & Black (ACB) youths and families. The RN will prioritize the health needs of Durham’s priority population, including Newcomers and African, Caribbean, and Black (ACB) communities, engaging both internally and externally to assess population health needs and enhance well-being through targeted assessment, treatment, and prevention strategies aimed at reducing health inequities.
