Community-Based Nursing With Purpose

Pyramid Healthcare’s Charlotte Assertive Community Treatment Team (ACTT) is seeking a compassionate and motivated Registered Nurse (RN) to provide high-intensity behavioral health nursing services directly within the community.

This position is ideal for nurses looking to step away from traditional bedside settings and into a rewarding, field-based role where they can build meaningful relationships, promote recovery, and make a lasting impact in the lives of individuals living with severe mental illness.

Our ACTT program operates as a “hospital without walls,” delivering comprehensive, community-based mental health services to individuals who need intensive support and may otherwise struggle to access care.

Key Responsibilities

  • Coordinate client care and collaborate with physicians, providers, and treatment team members

  • Monitor medication compliance and administer IM injections as prescribed

  • Manage medication inventory, refills, and nursing supplies

  • Record vital signs and assist providers during client appointments

  • Complete AIMS assessments upon admission and every six months thereafter

  • Provide medication education to clients, families, and ACTT staff

  • Transport clients to medical appointments and community resources as needed

  • Deliver nursing services directly in community settings throughout the Charlotte area

  • Participate in rotating crisis response and on-call coverage, including:

    • After-hours assessments

    • Hospital diversion support

    • Commitment procedures

  • Maintain accurate documentation and compliance with program standards

  • Perform additional duties as assigned

Required Qualifications

  • Active Registered Nurse (RN) license in North Carolina

  • Valid driver’s license with the ability to travel locally throughout the community

  • Ability to work independently while collaborating within a multidisciplinary team

  • Strong communication, organization, and critical thinking skills

  • Behavioral health, psychiatric, or community nursing experience preferred, but not required
