Are you an experienced Registered Nurse looking to take your career to the next level in a high-touch, concierge home care environment?
Affirmed Home Care is a leading private-pay home care agency dedicated to providing exceptional, personalized care to our clients. We are seeking a Registered Nurse Care Manager to join our growing team and oversee a caseload throughout Manhattan.
This is a 75% field-based position and is ideal for an experienced RN who is comfortable working independently, managing a caseload, supporting caregivers, and ensuring the highest standards of client care.
What You’ll Do
- Conduct comprehensive client assessments and ensure the highest quality of care
- Develop, implement, and update individualized care plans
- Obtain necessary physician orders for home care services in accordance with New York State regulations
- Monitor clients and report changes in their medical or mental condition to the attending physician and appropriate members of the care team
- Conduct supervisory visits and provide ongoing support to home care staff
- Collaborate with clients, families, caregivers, physicians, and the Agency’s care team
- Help ensure care plans are followed and client needs are appropriately addressed
- Support client safety, independence, and quality of life in the home environment
- Maintain accurate and timely clinical documentation
- Travel throughout Manhattan to conduct field visits and support clients and caregivers
What You Bring
- Current New York State RN license in good standing
- BSN preferred
- Valid BLS certification
- Minimum 2 years of recent clinical home care experience
- Hands-on clinical experience with advanced care, including:
- Ventilator management
- Tracheostomy care
- G-tube/PEG tube care
- IV infusions
- Wound care
- Strong clinical assessment and critical-thinking skills
- Excellent communication and interpersonal skills
- Strong organizational and time-management abilities
- Ability to work independently and effectively manage a field-based caseload
- Experience supervising and supporting field staff
- Strong attention to detail and a client-first approach
