Some of the most meaningful nursing happens in a living room, not a hospital hallway.

At Auburn Crest Primary & Palliative Care, our RNs go to patients where they live, caring for people managing chronic and serious illness. You will carry a manageable caseload, know your patients and their families by name, and have real say in the plan of care. If you have felt rushed past the parts of nursing that matter most, this is the role where you get them back.

What you will do

• Complete thorough assessments in the home and reassess at every visit
• Build and update each patient's individualized plan of care with our Medical Director and interdisciplinary team
• Coordinate care across disciplines so nothing falls through the cracks
• Manage symptoms, pain, medications, and wound care
• Teach patients, families, and caregivers what is happening and why
• Provide clinical direction to LPNs and aides, including supervisory visits
• Support families through the hardest moments with steadiness and compassion

What you bring

• Active, unencumbered Idaho RN license
• One year of nursing experience, with home health, hospice, palliative, or geriatric experience preferred
• Current CPR/BLS certification
• Valid driver's license, current auto insurance, and comfort working independently in a variety of home settings
• The kind of presence that makes a patient's worst day a little easier
