About the Role
As a Registered Nurse (RN), you'll partner closely with the Director of Nursing to oversee the medical care of individuals served across residential settings. You'll travel to assigned homes, provide skilled nursing care, coordinate healthcare services, educate staff, and ensure individuals receive high-quality, person-centered medical support.
This position is ideal for an RN who enjoys autonomy, building relationships, and working collaboratively with medical, clinical, and operations teams.
What You'll Do
• Assess individuals upon admission and complete ongoing nursing assessments
• Administer medications, vaccinations, and physician-ordered treatments
• Monitor health conditions, infections, and chronic medical needs
• Coordinate medical, psychiatric, dental, and specialty appointments
• Provide diabetic foot assessments and first aid as needed
• Triage medical concerns and communicate with physicians and psychiatrists
• Educate Direct Support Professionals (DSPs) on medication administration, infection prevention, and proper use of medical equipment
• Conduct medication room, eMAR, and EHR audits
• Assist with care planning and policy development
• Support infection prevention initiatives and health education
• Ensure compliance with HIPAA, state regulations, and company policies
• Serve as a clinical resource to interdisciplinary teams
Qualifications
• Active Registered Nurse (RN) license
• Minimum of two years of nursing experience preferred
• Valid driver's license with the ability to travel locally between residential locations
• Strong assessment, communication, and critical thinking skills
• Experience working with individuals with developmental disabilities or behavioral health needs is a plus
• Proficient with electronic medical records and computer systems
• Must successfully pass required background screenings
