ChristianaCare is hiring a full-time Registered Nurse, Renal Transplant Coordinator for our Kidney Transplant Department in Newark, DE.

The Kidney Transplant Program at ChristianaCare offers the only adult kidney transplant program in Delaware and is looking for a Renal Transplant Coordinator to join their team. The care provided encompasses kidney transplant education, evaluation, waitlist management, and after transplant care, as well as living donation.

The Transplant Coordinator is a vital member of the multidisciplinary transplant team, serving as the primary point of contact for transplant candidates, living donors, physicians, dialysis and nephrology providers, and hospital personnel throughout the transplant process. This role coordinates all clinical aspects of care, including patient evaluation, transplant readiness, and UNOS listing, while ensuring a seamless and compassionate experience for patients and their families from referral through post-transplant follow-up.

Working collaboratively with physicians, surgeons, nurses, social workers, financial coordinators, and administrative staff, the Transplant Coordinator promotes high-quality patient care, facilitates access to services, and supports successful transplant outcomes. The coordinator also serves as a clinician, educator, consultant, and leader, utilizing evidence-based practice, critical thinking, and clinical expertise to address complex patient needs and advance excellence in transplant care

Work Schedule:

  • Monday through Fridays: 8am to 4:30pm

Duties / Responsibilities:

The Registered Nurse Transplant Coordinator serves as a key clinical liaison throughout the transplant journey, providing comprehensive care and coordination for patients before and after solid organ transplantation. This role collaborates closely with a multidisciplinary team to manage patient evaluations, transplant readiness, post-transplant follow-up, care transitions, and ongoing clinical support across both inpatient and outpatient settings. Responsibilities include coordinating diagnostic testing and procedures, monitoring patient outcomes, facilitating communication among patients, families, providers, and external organizations, and ensuring compliance with OPTN, CMS, and Joint Commission regulations. The coordinator also provides 24/7 on-call support, including transplant coordination when donor organs become available, and assists with discharge planning and resource referrals to support optimal patient outcomes.

Education Requirements:

  • B.S./BSN in Nursing required, MSN preferred.
  • A minimum of three (3) years of registered nursing experience is required, preferably in Transplantation, Critical Care, Case Management, Nephrology, Dialysis, or a related specialty.
  • Basic Life Support (BLS) required

Hourly Pay Range: $42.11 - $67.37

This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
