Description:

The Pre Liver Transplant Coordinator assists in the pre-transplant phase of the patient's evaluation. Reviews diagnostic tests, laboratory work, and other pertinent information in clinic and before medical review board. Coordinates care of the patient, triages calls, and assists in medical and medication management.

What You Will Do

  • Coordinate pre-transplant evaluations for recipients and living donors.
  • Manage post-transplant follow-up care for transplant recipients and donors.
  • Educate patients and donors on all aspects of the transplant process.
  • Review diagnostic tests, laboratory results, and clinical information for transplant eligibility and review board presentations.
  • Coordinate interdisciplinary care and treatment plans.
  • Triage patient and donor calls and assist with medication management.
  • Participate in on-call organ procurement and donor offer activities.
  • Ensure compliance with regulatory and accreditation requirements (UNOS, CMS, Joint Commission, insurance carriers).
  • Prepare quality improvement reports and support transplant program administration.
  • Attend transplant-related meetings, conferences, and review activities.

What You Will Need

  • Bachelor's degree in nursing (BSN)
  • Current Registered Nurse (RN) license in the State of Illinois
  • Current CPR/BLS Certification
  • 3 to 5 years of nursing experience in hepatology, gastroenterology, ambulatory care, care coordination, or a related specialty strongly preferred.

Compensation:

Salary Range: $39.48 - 65.14 per hour

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
