Key Responsibilities:

• Provide on-site patient care using a harm reduction approach
• Focus on HIV, Hepatitis C, and chronic condition management
• Conduct assessments, diagnoses, and treatment planning
• Prescribe and manage medications
• Educate patients on prevention and wellness
• Collaborate with physicians and interdisciplinary teams
• Maintain accurate documentation in EMRs
• Follow CNO standards and privacy regulations
• Engage local health and community organizations

Qualifications:

• Registered NP (Adult or Primary Health Care) with active CNO license
• Masters degree (MN-NP or MScN)
• Preferred: experience in harm reduction, HIV/Hep C, or community health
• Strong clinical decision-making and patient communication skills
• Proficient in EMRs
• Current N95 Mask Fit Test and immunizations (MMRV, TB test, or chest X-ray)
