The North Central Health District’s Perinatal Health Home Visiting Program is a warm, community-centered initiative designed to bring high-quality healthcare, education, and support directly to families in the rural areas of our district. Our team works closely with expectant and new parents, helping them navigate pregnancy, birth, and early childhood with confidence. By meeting families where they are, we build meaningful relationships, reduce barriers to care, and make a real difference in maternal and child health outcomes. Under supervision, provides professional nursing care in accordance with nurse protocols, policies, and procedures, while playing a critical role in promoting maternal and child health, reducing disparities in birth outcomes, and supporting overall community wellness.
This position is based at the North Central Health District’s Main Office in Macon, GA, and requires travel to provide home visits across Baldwin, Bibb, Hancock, Jones, Peach, Washington, and Wilkinson counties, with occasional visits to additional counties such as Crawford, Houston, Jasper, Monroe, Putnam, and Twiggs. This will change as the program expands. Candidates should reside within a reasonable commuting distance and be willing to travel to locations throughout our district as needed. Employees in this role are eligible to receive mileage compensation for work-related travel. This is an excellent opportunity to work directly with communities across the district and make a meaningful impact.
Associate's degree in nursing from an accredited college or university and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification. Must have a valid driver’s license and reliable transportation
The DPH Commissioner and agency leaders encourage all employees to engage in regular wellness activities and to make lifestyle choices that promote health and well-being.
The use of physical activity breaks during the workday is authorized to support this philosophy and help employees meet their wellness goals. A maximum of 30 minutes of physical activity may be allocated per workday, typically in the form of two 15-minute breaks or one 30-minute break.

DPH accepts educational credentials recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) as valid. DPH will contact educational institutions to verify degrees, diplomas, licensures, and other relevant credentials.
