The Patient Flow team offers a unique opportunity to apply your critical thinking skills in a fast-paced, highly collaborative environment—without the physical demands of the bedside.

In this role, you will have a direct impact on patient care across all three Sanford Fargo campuses. You will help ensure patients receive the right care, at the right time, in the right location. Your knowledge of inpatient workflows and clinical acuity will be essential as you coordinate admissions, transfers, and patient flow across the North Region.

As part of the Patient Flow team, you will serve as the central point of contact for providers. You will manage incoming calls, coordinate patient transfers, and align admission activity. You will also monitor real-time and projected bed capacity to support safe and efficient operations. This role requires strong clinical judgment, quick decision-making, and the ability to anticipate care workflows.

Sign on bonus available for eligible candidates!The Patient Placement leader is responsible for continuity of operations within the facility by overseeing patient admissions and transfer activity while monitoring bed/patient care capacity both currently and prospectively. Responsible for facilitating continuous throughput of patients from all points of entry by utilizing effective verbal and written communication; must be able to make efficient, effective clinical decisions; and coordinate and deploy ancillary services to needed areas.The patient placement leader is the liaison between clinical and ancillary departments. Responsible for evaluating patient status, reviewing patient chart regarding current clinical conditions, and determining placement of patient based on patient history and stability. Requires flexibility, ability to prioritize multiple tasks at one time and effectively manage conflict and stress. Requires a thorough understanding of organization. Current knowledge related to departmental policies and each department's clinical practice. Must have current understanding of regulations and scope of practice for various roles. Connect with clinical leaders to assess patient flow. Coordinate the placement of all admissions (elective and emergent) and transfers in accordance with the clinical standards and guidelines. This position will also be responsible for directing patient admissions to the most appropriate bed available using criteria for admission. Providing oversight of patient throughput to minimize delays in bed turnaround times. Planning for required staffing resources needed to support patient placements. Make appropriate patient placement decisions using admission guidelines that define the level of care each unit can provide and specialty care availability Uses proactive demand capacity planning to project needed beds and staffing for the number of patients expected to present for admission. Communicates regularly with timely updates to ensure optimal patient flow and address throughput barriers. Utilizes data measurement and patient flow event information to drive for improved results. Participates in review of staffing effectiveness in relation to impact on patient flow. Mentors and assists nursing staff to maximize proficiency and timeliness in the admission, transfer and discharge processes. Reviews patient flow events and participates in root cause analysis and improvement activities.

Qualifications

Bachelor's degree in nursing preferred. Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).

Minimum of three years’ clinical experience required.

Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification required within six months of employment. Re-certification as required.
