This is not an a remote position, this position is located on site at the Transfer Center-1300 Old Denbigh Blvd | Newport News, VA 23602.
The Patient Flow Coordinator acts as a liaison between nursing, bed control and various departments throughout the hospital to coordinate and ensure prompt and accurate placement and assignment of patient into a bed/unit. Facilitates resolution of any issues that affect efficient patient flow throughout the facility. Manages patient throughput from all points of entry to facilitate overall hospital patient flow by collaborating with the medical staff and other members of the interdisciplinary team in the coordinating, organizing, and planning, of patient movement within the medical center. Monitors bed capacity, staffing beds, and the demand for beds daily and communicates action plan for patient movement. Collaborates with transfer center, hospital staff, case management, and providers to eliminate barriers which may impede patient flow. Provides day-to-day reports on bed availability to the clinical and administrative staff. Provides direction and support to bed management to ensure efficient patient access and flow.
