One of a network provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking 1 FTE permanent Registered Nurse - Palliative Carelink. As a member of NorWest Community Health Centres, the Community-Based Palliative Carelink program, the Registered Nurse plays a vital role in delivering holistic, compassionate, and equitable care to clients with palliative needs across the community. Guided by NorWest Community Health Centres mission to work with the community for the community, this role prioritizes responsive, client-centered care and seamless coordination across multiple care settings including homes, shelters, outreach programs, and health system partners. This role serves as a central contact for palliative carelink clients, caregivers, and health care providers, supporting individuals throughout their illness trajectory with an emphasis on dignity, respect, and cultural sensitivity. The nurse will provide leadership in system navigation, community engagement, and care integration, supporting timely access and continuity of care.

The position has a competitive salary, the Health Care of Ontario’s Pension Plan (HOOPP), and an excellent group health benefits package.

The salary range for this position is $72,039 to $83,187 per year. We do not use AI to screen resumes.

QUALIFICATIONS:
• Registered Nurse in good standing with the College of Nurses of Ontario
• Minimum 2 years of experience in palliative care nursing, preferably in community settings
• Completion of LEAP (Learning Essential Approaches to Palliative and End-of-Life Care) is required
• CNA certification
◦ Experience Hospice Palliative Care Nursing/CHPCN(C)- an asset
◦ Experience Oncology Nursing/CON(C) - an asset
◦ Experience Community Health Nursing/CCHN(C) - an asset
• Demonstrated ability to work in outreach or mobile care settings
• Strong knowledge of palliative care principles and symptom management
• Experience coordinating care across healthcare and community support systems
• Deep understanding of the social determinants of health and working with marginalized populations
• Experience delivering culturally safe care to Indigenous and equity-deserving communities
• Excellent communication, interpersonal, organizational, and leadership skills
• Proficient in electronic medical records and Microsoft Office
• Valid driver’s license and access to a vehicle are required
