Job Description

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Schedule: PRN- coverage as needed (5-hour shifts - 10 hours per week). Weekdays will vary. Some weekend coverage as well. Likely no more than 40 hours a month.

An employee in this position will manage referrals to The Organization, coordinating the process through the admission of the patient while managing referral databases.  This employee will also be performing telehealth triage functions including but not limited to receiving calls; providing phone support; providing interventions utilizing BCN policies, procedures, and protocols; dispatching on-call nurses as needed, completing documentation utilizing the EMR, point of service and making appropriate notifications to primary team regarding any changes in LOC.
Position Qualifications:
• Registered Nurse or Licensed Practical Nurse with a minimum of three years of Hospice, home health, Palliative, Primary Care or critical care/ED experience preferred.  Telehealth experience also preferred.
• Ideal candidate will demonstrate strong communication skills and customer service mindset.
• Ability to exercise judgment and discretion in confidential matters.
• Has excellent assessment, nursing judgment, and communication skills.
• Must demonstrate exceptional interpersonal relationship skills necessary to interact in a positive, compassionate, and professional manner with all types of callers (i.e. patients/clients families, referral sources, partners, and BCN team members).
• Demonstrated ability to respond to the needs of patients, families, and referral sources in varied settings.
• Able to cope with heightened family emotional stress, and tolerant of individual lifestyles.
• Ability to partner with multiple clinical teams to ensure excellence in customer service.
• Must have strong working knowledge of phone and computer equipment.
• Is self-directed with the ability to work with little supervision.
• Is flexible and cooperative in fulfilling role obligation and has the ability to work with an interdisciplinary team.
• Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
• Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
