Job Description

IU Health is seeking a compassionate and dedicated Registered Nurse Navigator to join our Palliative Care team. This full-time, day shift role (Monday-Friday) provides an opportunity to deliver personalized, patient-centered care that aligns with IU Health’s mission of improving the health of our communities. The ideal candidate will have previous experience in palliative care or hospice, demonstrating a strong commitment to patient advocacy, interdisciplinary teamwork, and quality care delivery.

Key Responsibilities:

  • Coordinate comprehensive care needs of assigned IU Health patients, developing individualized plans that ensure seamless delivery across the entire care continuum.
  • Partner with patients, families, and primary physicians within IU Health to create tailored care strategies based on individual preferences, clinical needs, and goals.
  • Collaborate effectively with IU Health physicians, nurses, social workers, and other healthcare professionals to facilitate timely and appropriate tests, treatments, and interventions.
  • Advocate fiercely for patients’ needs, ensuring their voices are heard and their dignity maintained throughout their care journey.
  • Balance clinical care requirements with financial considerations to promote efficient, effective, and sustainable treatment plans within IU Health’s standards.
  • Maintain accurate documentation and ensure compliance with IU Health’s medical record requirements, healthcare regulations, and policies.
  • Stay informed about current best practices, medical terminology, and healthcare regulations relevant to palliative and hospice care within IU Health.

Qualifications:

  • Associate's Degree in Nursing (ASN) required; Bachelor of Science in Nursing (BSN) preferred
  • Active Registered Nurse (RN) license in Indiana or an active Nurse Licensure Compact (NLC) RN license
  • Graduation from a nationally accredited nursing program
  • 3-5 years of clinical experience in palliative care or hospice highly preferred
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred
  • Basic Life Support (BLS) certification through the American Heart Association (AHA) required

  • Proficiency in personal computer use, including word processing, spreadsheets, and database management
  • Strong understanding of medical terminology, medical record requirements, and healthcare regulations relevant to IU Health
  • Excellent communication, interpersonal, and interdisciplinary collaboration skills
