The Centre for Complex Diabetes Care (CCDC) is a Ministry-funded program within Ontario’s diabetes care system, providing specialized, short-term, interprofessional care for individuals with complex diabetes and associated comorbidities. The program supports patients with high clinical needs through comprehensive assessment, intensive case management, education, and care coordination.

Using a team-based, patient-centered approach, the CCDC focuses on stabilizing medical conditions, enhancing self-management, and reducing hospital utilization. The program works in close collaboration with primary care and community services to ensure seamless transitions across the continuum of care.

At Trillium Health Partners, the CCDC delivers high-quality, evidence-based care to a diverse patient population across the Mississauga and Oakville region.

Principal Responsibilities:

  • Collaborate effectively with nursing colleagues, physicians, allied health professionals, and patients/families to deliver advanced nursing care, including comprehensive physical and psychosocial assessments, development of individualized outpatient care plans, and case management.
  • Develop and implement nursing care plans and provide counselling for patients with diabetes and multiple comorbidities, including renal disease, cardiovascular disease, and mental health conditions.
  • Foster strong partnerships with physicians and interprofessional team members to ensure coordinated, high-quality care for patients and their families.
  • Apply advanced theoretical knowledge and approved medical directives to independently and collaboratively assess and manage patient care needs across the continuum, demonstrating strong clinical judgment and decision-making skills.
  • Provide clinical leadership through mentorship and participation in formal and informal staff education initiatives.
  • Maintain accurate, timely documentation and ensure effective communication with all members of the patient’s circle of care.
  • Evaluate patient outcomes and care delivery, including the collection and maintenance of accurate data for reporting and quality improvement purposes.
  • Utilize evidence-based teaching strategies grounded in adult learning principles to support patient education, engagement, and behaviour change (e.g., motivational interviewing, stages of change model).
  • Develop and adapt educational materials to meet the diverse needs of patients and families.
  • Contribute to program planning, implementation, and evaluation activities.

Key Qualifications:

  • Current registration in good standing with the College of Nurses of Ontario.
  • Minimum of five (5) years’ experience in diabetes management; Certified Diabetes Educator (CDE) designation preferred.
  • Demonstrated expertise in conducting comprehensive assessments of patients with complex medical needs.
  • Strong clinical knowledge of Type 1 and Type 2 diabetes and associated comorbidities across the lifespan.
  • Experience with insulin pump therapy and continuous glucose monitoring systems; certification as a product trainer is considered an asset.
  • Broad knowledge of community resources and the ability to apply critical thinking and problem-solving skills to support optimal patient outcomes.
  • Proven critical thinking, clinical reasoning, and decision-making abilities.
  • Excellent interpersonal and communication skills, with the ability to build effective relationships with patients, families, healthcare providers, and community partners.
  • Proficiency in electronic health documentation systems (e.g., EPIC) and Microsoft Office applications.
  • Demonstrated reliability, including a strong attendance record.
  • Current Basic Cardiac Life Support (BCLS) certification.
