We are currently seeking a Registered Nurse or Enrolled Nurse to join our team at Kedron Medical Centre. This is an exciting opportunity to play a key role in Chronic Disease Management, working closely with GPs and patients to improve health outcomes through preventative care and ongoing support.

About the Role

In this role, you will:

• Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs)
• Support patients in managing chronic health conditions and achieving positive health outcomes
• Conduct health assessments, including preventative health and chronic disease screening activities
• Work closely with GPs, specialists and the wider multidisciplinary healthcare team
• Provide patient education, coaching and ongoing support to encourage positive health behaviours
• Coordinate referrals to allied health professionals and external healthcare providers
• Perform clinical services including ECGs, Spirometry, ABI testing and Urinalysis where appropriate
• Utilise tools such as POLAR, PenCAT and HotDocs to identify eligible patients
• Maintain accurate clinical records and appointment administration
• Support preventative health initiatives and proactive patient outreach programs

About You

To be successful in this role, you will have:

• Current AHPRA registration as a Registered Nurse or Enrolled Nurse
• Previous experience in Chronic Disease Management highly regarded
• Experience in General Practice or Primary Healthcare preferred
• Strong communication and interpersonal skills
• Excellent organisational and time management abilities
• A proactive, patient-focused approach to care
• The ability to work both independently and collaboratively within a team
• A passion for preventative healthcare and improving patient outcomes
