
The RN Intake Coordinator serves as the first clinical point of contact for home health referrals, conducting telephone assessments to determine patient eligibility and coordinating safe, timely admissions. This role partners closely with referral sources and the interdisciplinary team to ensure seamless patient placement while delivering exceptional service and supporting high-quality care transitions.

Compensation and Benefits:
Pay Rate: Competitive salary based on experience.

Minimum Qualifications
Current license to practice as a Registered Nurse in state of practice. Valid compact licenses must be transferred to the state of practice within 60 days of the start of employment.
Basic Life Support Certification (BLS) for healthcare providers.
One year acute patient care or homecare experience.
Excellent customer service skills.
Must be able to work independently, prioritize and perform detailed work in a fast paced team environment.
Ability to exercise courtesy, tact, and sensitivity with team members and customers.
Bachelor's degree in nursing (BSN)
RNs with less than 12 months of experience must obtain their Bachelor of Science in Nursing (BSN) within four (4) years of their hire date.
Preferred Qualifications
Bachelor's degree in nursing (BSN). Education must be obtained from an accredited institution. Degree will be verified.
Homecare, insurance or other third party payer experience.
Prior leadership experience.
