Job Description

The Hospitalist Coordinator RN plays a key role in supporting safe, efficient, and patient-centered care for hospitalized patients. This position serves as a clinical liaison between physicians, nurses, care managers, patients, and families to coordinate care, support communication, improve patient flow, and facilitate successful transitions throughout the hospital stay.

Combining clinical expertise with care coordination and patient advocacy, this role helps ensure exceptional outcomes and experiences for patients while supporting operational excellence across the Hospital Medicine service.

Shift Details:

  • PRN position with a commitment of 8 shifts per 4-month scheduling block
  • Coverage responsibilities include:
  • 2 shifts supporting LDS Hospital
  • 3 shifts supporting Tele-Hospitalist Services
  • 3 shifts supporting Tele-Emergency Medicine (Tele-EM)

Preferred Experience

  • Three or more years of acute care nursing experience preferred
  • Background in Medical/Surgical, ICU, Emergency Department, or other inpatient hospital settings strongly preferred

What You'll Do

  • Coordinate daily patient assignments, census management, and workflow support for hospitalist providers.
  • Review patient information prior to rounds, identify discharge barriers, and collaborate with care management and ancillary teams.
  • Participate in interdisciplinary rounds to facilitate communication among providers, nurses, patients, and families.
  • Support physician rounding activities by coordinating care plans, follow-up needs, and transitions of care.
  • Assist with discharge planning, follow-up appointments, equipment needs, and continuity of care arrangements.
  • Serve as a liaison between patients, families, providers, pharmacies, and community resources.
  • Promote patient engagement through education, care coordination, and post-discharge support.
  • Support patient safety, quality improvement, operational efficiency, and patient experience initiatives.

Minimum Qualifications

  • Current Registered Nurse (RN) license in the state of practice.
  • Basic Life Support (BLS) certification for Healthcare Providers.
  • Two years of clinical hospital experience.
  • Basic computer proficiency.
  • Strong verbal and written communication skills.
  • RNs hired or promoted into this role must obtain a Bachelor of Science in Nursing (BSN) within three years of hire or promotion if not already completed.
  • Bachelor of Science in Nursing (BSN) from an accredited institution. Education will be verified.

Preferred Qualifications

  • Experience in hospital-based care coordination, case management, utilization review, or hospital medicine.
  • Knowledge of patient throughput, discharge planning, and interdisciplinary care coordination.
  • Experience with quality improvement, process improvement, or clinical operations initiatives.
  • Familiarity with electronic medical records and healthcare information systems.

Physical Requirements

  • Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs.
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, etc.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$41.20 - $62.17
