Job Type: Full-Time, In-Person
Location: Porterville, California area
Compensation: $50+/hour plus benefits

Job Responsibilities:

• Provide high-quality Home Health care to a caseload of approximately 4 patients daily
• Conduct patient assessments, monitor vital signs, and develop care plans
• Assist with diagnostic evaluations and test result interpretations
• Collaborate with Medical Assistants, Nurse Practitioners, and administrative teams
• Offer triage services to determine the urgency of care
• Deliver specialized care for patients with memory-related conditions
• Maintain thorough and accurate patient records

Job Requirements:

• Active RN license in California
• Home Health or Hospice experience preferred
• Must have a reliable vehicle for travel
• Strong assessment, communication, and critical-thinking skills
• Ability to work collaboratively in a multidisciplinary team
