Job Title: Registered Nurse — Home Health

Location: Irving, TX — Field-based

Type: Full-Time | On-site

Positions Available: 3

We are seeking experienced Registered Nurses for home health field positions across the Texas market. Candidates will deliver skilled nursing care to patients in their homes, managing complex medical conditions and coordinating with physicians and care teams to ensure optimal patient outcomes.

Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care
• Administer medications, wound care, IV therapy, and other skilled nursing interventions
• Educate patients and families on disease management, safety, and self-care
• Coordinate with physicians, therapists, and social workers on patient care plans
• Document all patient interactions accurately in compliance with Medicare and agency standards
• Monitor and report changes in patient condition to supervising clinicians

Requirements

• Active RN license in state of practice required
• Minimum 1 year of home health or acute care nursing experience preferred
• OASIS certification or experience preferred
• BLS/CPR certification required
• Valid driver's license and reliable transportation required
• Strong clinical assessment and documentation skills
