Job Title: Registered Nurse — Home Health

Location: Sterling Heights, MI — Field-based

Type: Full-Time | On-site

We are seeking experienced Registered Nurses for home health field positions serving patients across Macomb, Oakland, and Wayne counties in Michigan. Candidates will provide skilled nursing care in the home setting, coordinating with physicians and interdisciplinary care teams to deliver high-quality, patient-centered outcomes.

Key Responsibilities

• Conduct comprehensive home health assessments and develop individualized plans of care
• Administer medications, wound care, IV therapy, and skilled nursing interventions
• Educate patients and families on disease management, medication compliance, and safety
• Complete OASIS documentation accurately and in compliance with Medicare standards
• Coordinate with physicians, therapists, and social workers on patient care plans
• Report changes in patient condition to supervising clinician promptly

Requirements

• Active Michigan RN license required
• Minimum 1 year of home health or acute care experience preferred
• OASIS experience preferred
• CPR/BLS certification required
• Valid Michigan driver's license and reliable transportation required
