• Location - travel to see clients in Wagga Wagga and surrounding areas
• Permanent Part Time (3 days)
• Competitive Pay rate – up to $59.00 per hour (based on experience) + superannuation + km reimbursement
• Up to $18,550 tax free with Not for Profit Salary Packaging

Are you a compassionate and skilled nurse with a strong interest in enabling older Australians to remain in their own homes? We’re seeking a committed Community Nurse to join our team and help improve the lives of older adults living independently in the community.
In this rewarding role, you’ll draw on primary health care principles to deliver holistic, person-centred care to older adults experiencing chronic conditions and complex needs. Working as part of a multidisciplinary team, you'll provide early intervention, health education, and recovery-focused support within clients' homes and community settings.

Day to day this role looks like:
• Providing direct clinical nursing care to our clients in their homes in line with best practice nursing procedures
• Conducting clinical assessments, dressings, medication assessment and catheter changing
• Developing a holistic nursing care plan in collaboration with client, carer, Care Advisor and Allied health where relevant
• Providing safe, quality nursing practice to clients with specific needs and/or complex needs eg. Wound care, palliative care, education and support
• Liaising with all parties involved in the care of the client to maintain holistic health care and wellbeing
• Ensuring all documentation is accurate and completed in a professional and timely manner
• Working in a safe manner to protect yourself, your colleagues and clients closely following all policies and procedures
To be successful in this role you’ll need
• A genuine passion for providing high quality care to the elderly
• Current AHPRA registration following completion of a Bachelor of Nursing or similar
• Minimum 3 years post graduate nursing experience
• Experience in clinical assessments
• Initiative and the ability to work independently
• Strong computer skills especially Microsoft Office products and ability to pick up new systems
• NSW Drivers license and a reliable vehicle
