Hours of Performance: Services shall be provided Monday through Friday, between the hours of 0700 and 1600 hours.

-Assist in defining patient population in the Medical Home Port (MHP) Team requiringprofessional nursing intervention and use evidenced based clinical practice guidelines to evaluateand provide nursing care for selected patients.-Work directly with selected patients and families to clinically direct, plan, coordinate, implementand evaluate a plan of care to meet needs. Collaborate with other health care disciplines toprovide total quality patient care. Communication avenues may include, but not all inclusive ofasynchronous messaging to improve communication, telephone consultations and other nontraditional means.-Function as care coordinator to assure all aspects of patient care needs are met, to includeassessment, intervention and follow-up in screening of patient/patient problems.-Initiate and maintain pertinent patient and family medical history on patients, recordingobservations and stated symptoms for use by the Medical Home Port Team. Observe and recordpertinent vital signs, determine and order laboratory procedures on patients that may be requiredby the provider to diagnose and treat the patient, per command and department policies. Orientnew patients and their families to clinic policies as required-Take appropriate action such as practitioner referral and evaluation, health care advice, initiationof medical protocols or referral based on medical staff protocols.-Proactively coordinate the screening of their enrolled patients to identify patients needingpreventative services and education based on their specific health care needs including, but notlimited to health and wellness topics, medical conditions and medical procedures. (Examplesinclude mammograms, A1C, etc.)-Ensure review of emergency treatment records (ETR) and follows up with patient, per commandand department policies.-Track Health care Effectiveness Data and Information Set (HEDIS) measures, lab/rad results andnotify patients per command/department policies. Check patient identification, Health care-Provider’s orders, and transfer information as applicable. Arrange for ordered diagnostic andtherapeutic services as directed.-Administer oral parenteral, intravenous (IV) medications, and maintains annual competencies asoutlined in departmental policies.-Actively participate in MHP Team huddles. Proactively assist team’s identification of diagnosticprocedures required during schedule scrub. Assist with identification of patient education needsand review plan with team members for coordination of educational needs 5.5.3.2.6.11. Providetechnical direction to licensed practical nurses (LPNs), medical assistants and Hospital Corpspersonnel.-Administer specific medications at designated times as ordered.-Observe patients and report pertinent data on condition and medication or treatments.
-Prepare and administer IVs and monitor their proper function.-Demonstrate proficiency according to operating manuals when using patient care electricalequipment.-Chart nursing observations and ensure record is complete with all procedures and test resultsrecorded. Maintain required clinic records, patient charts, forms, logs, records, registers, andcollect required statistics.-Perform nursing techniques for the comfort and wellbeing of patients.-Prepare equipment and assist health care providers with treatment procedures.-Recognize emergencies and respond with appropriate protocol, e.g. resuscitation, oxygen,emergency drugs, cardiac massage, application of medical equipment, as established by themedical staff.-Explain procedures and treatments to patients and significant others to allay apprehension andgain cooperation and support.-Assist in instructing LPNs, nursing assistants and ancillary personnel; and ensure proper andcomplete discharge of Health care Provider’s orders on patient care and treatment.-Report to Government supervisor when unit supplies, equipment, linens, medications, drugs, etc.are needed. Maintain emergency supply cart and ensure that equipment is clean and in goodworking order prior to use. Report discrepancies to the department head or supervisor.-Participate in the peer review and training of staff and maintenance of the Medical Home PortTeam Accreditations (National Committee for Quality Assurance (NCQA), Joint Commission,Inspector General (IG), etc.).
