About the Position:

  • Full Time: 1.0 FTE (80 hours per pay period)
  • Monday-Friday
  • Clinic Nursing Services
  • Training for this position will be onsite.

*Following training, the position is hybrid-remote. Your home office must meet Safety, Privacy, and Internet Speed requirements set forth by Grand Itasca Clinic & Hospital to be eligible for this position.

Under the supervision of the Clinic Nursing Leadership, the registered nurse supports primary care providers in Electronic Health Record (e.g. MyChart) message management. Working close in collaboration with the Clinic Medical Director and primary care department physician leadership to optimize EHR message management through the electronic health record (EHR) system to support the ever-growing population of patients who rely on EHR messaging to access health care. The RN will be the first line of contact to patients in response to EHR messages responding to all EHR messages by offering clinical guidance, triage to higher level of care, and patient education prior to heightening to provider level, if indicated.

What you'll do in this role:

  1. Utilizes the nursing processes to provide holistic, compassionate, safe, high quality, and population-based care.
  2. Serves as an advocate and resource to patients who seek care via EHR messaging.
  3. Reviews all in-basket messages for all primary care providers daily (whether providers are in clinic or off) to "mature" each message before routing to provider or covering provider, when indicated.
  4. Turn EHR messages into triage encounters when indicated as part of "maturing" the encounter.
  5. Meet up to weekly for with clinic medical director or designated physician "advisor" for education on case review and to share insight on effective management of in-baskets.
  6. Collaborates with Behavioral Health Clinician to coordinate behavioral and mental health patients who frequently utilize EHR messaging to drive care management.
  7. Collaborates with clinic care coordination team to develop plans to best assist patients with high EHR messaging utilization.
  8. Provide patient education and care coordination on best ways to seek care through supported messaging platforms.
  9. Provides evidence-based education to patients/families related to clinical care.
  10. Makes follow up patient phone calls to ensure patient feels confident in their care direction, stimulated from EHR message inquiries.
  11. Assists to coordinate follow-up appointments with primary or specialty care.
  12. Knows the key staff available to help patients with financial and emotional support; arranges for patients to meet with these resource people if needed.
  13. Serve as a contact for patients, providers and other customers.
  14. Other duties as assigned.

If you have these qualifications, we'd love to chat:

  1. Currently registered with the Minnesota Board of Nursing.
  2. Bachelor's degree preferred.
  3. Current BLS certification or ability to obtain within 2 months.
  4. Demonstrate clinical competence in providing direct patient care, patient and family education, and assisting the provider in the coordination of patient care.
  5. At least one year experience triaging of patients using clinical judgement within the RN scope required.
  6. Three years of experience working in the ambulatory interface of Epic EHR and care coordination in the health care environments preferred.
  7. Motivational Interviewing experience preferred.
  8. Skill in coordinating care needs.
  9. Must meet additional requirements to work hybrid-remote.
