Dunklau Gardens is our long-term care, home health & hospice and post-acute short-term skilled rehab care facility.

The practice of nursing by a registered nurse shall mean the assumption of responsibility and accountability for those nursing actions which include, but are not limited to: assessment, planning, implementation and evaluation of a nursing diagnosis. Execution of the prescribed diagnostic and therapeutic treatment regiments, administering, delegating, and evaluating nursing activities.

Responsibilities:
1. Provides an accurate and thorough assessment of the patient/family to identify human responses to actual or potential health conditions.
2. Formulation of a nursing diagnosis to identify and plan for the needs of an individual, family, or group.
3. Implementation of a nursing treatment regimen to provide for the selection, performance and management of proper nursing practices.
4. Evaluation of the plan of care an treatment for all patients under his/her care to promote resolution of patient problems and positive patient outcomes.
5. Coordinates and supervises patient care to provide for the management of a multidisciplinary approach to patient care.
6. Safety initiative - Complies with Methodist Fremont Health safety policies and procedures and regulatory requirements e.g., Occupational Safety and Health Administration (OSHA), Joint Commission (JCAHO), Clinical Laboratory Improvement Amendments (CLIA).
7. For Home Health: Understand and implement Medicare requirements in care of the home care patients to meet eligibility, documentation and compliance.
8. For Home Health: Coordinates care of home care patients to provide for Continuity of Cares.
