The Role:

Medcan is recruiting for a Nurse Consultant, RN to join our Annual Health Assessment team on a full-time, 12-month contract position covering a maternity leave.

The Annual Health Assessment is Medcan’s comprehensive preventive health assessment, bringing together physician review, nursing support, diagnostic testing, and coordinated follow-up to help clients better understand their health, identify risks early, and take clear next steps in their care.

We are seeking an experienced and client-focused professional with a great attitude who can work independently and as part of a team. The Nurse Consultant acts as a key point of contact, providing guidance, education, and support to clients before, during, and after their assessment.

This role works closely with a multidisciplinary team of physicians, nurses, allied health professionals, and coordinators to support timely communication of results, follow-up care, specialist referrals, and diagnostic imaging as required.

The Accountabilities:

• Serve as a primary nursing contact and trusted clinical resource for Annual Health Assessment clients, providing timely education, guidance, reassurance, and support before and after their assessment.
• Communicate and explain routine, abnormal, sensitive, and time-sensitive clinical results based on physician review and recommendations, ensuring clients understand their health information, follow-up plan, and next steps in care.
• Conduct nursing assessments and symptom triage using sound clinical judgment, critical thinking, and scope-appropriate decision-making to determine the urgency of client concerns and the appropriate level of follow-up.
• Provide individualized health education related to prevention, lifestyle modification, screening results, treatment plans, new or ongoing diagnoses, and recommended follow-up care.
• Support clients and their families through navigation of healthcare services, community resources, specialist referrals, diagnostic imaging, and investigative studies in collaboration with physicians, coordinators, Referrals, and Medical Records teams.
• Coordinate follow-up care, appointments, prescription renewal requests, case management activities, and ongoing client communication to support continuity of care after the Annual Health Assessment.
• Document all nursing assessments, client communications, results review, follow-up actions, and care coordination thoroughly, accurately, and in a timely manner within electronic medical records and client management systems.
• Collaborate closely with physicians, nurses, allied health professionals, coordinators, and membership colleagues to ensure a seamless, consistent, and high-quality client experience.
• Navigate and de-escalate complex client concerns with professionalism, empathy, and sound judgment, escalating clinical, operational, or client experience issues through the appropriate pathway when required.
• Deliver an elevated, hospitality-inspired client experience through personalized, high-touch interactions that anticipate client needs, build trust, and help clients feel supported, heard, and valued throughout their healthcare journey.
• Act as an AI-enabled healthcare professional by using intelligent technologies, automation, digital tools, and workflow supports to enhance service delivery and create more meaningful client interactions.
• Participate in rotational nurse-on-call coverage and support team priorities as required to maintain service continuity and exceptional client care.

The Requirements:

• Registered Nurse in good standing with the College of Nurses of Ontario.
• Current Basic Life Support (BLS) or CPR Healthcare Provider certification.
• Minimum 2-5+ years of nursing experience, preferably in case management, primary care, ambulatory care, community nursing, preventive health, acute care, or chronic disease management.
• Demonstrated clinical assessment, symptom triage, critical thinking, and decision-making skills, with the ability to recognize urgent or escalating concerns and take appropriate action within nursing scope.
• Strong understanding of professional nursing scope, accountability, documentation standards, confidentiality, and appropriate clinical escalation pathways.
• Exceptional written, verbal, telephone, and electronic communication skills with the ability to communicate complex clinical information clearly, accurately, and compassionately.
• Strong documentation skills with meticulous attention to detail and the ability to maintain accurate, thorough, and timely records within electronic medical records and client management systems.
• Excellent organizational and time management skills with the ability to prioritize effectively, manage competing client needs, and follow through on multiple care coordination activities in a fast-paced environment.
• High emotional intelligence, strong problem-solving abilities, and a client-focused, hospitality-driven approach to care, including the ability to navigate sensitive conversations and de-escalate complex client concerns.
• Experience using Electronic Medical Records, preferably Accuro, and Client Relationship Management systems such as Salesforce.
• Strong technical proficiency with Microsoft Teams, Outlook, Word, Excel, PDF editing software, web-based applications, internet browsers, and client portal technology.
• Comfort using AI-enabled tools, automation, and digital workflows responsibly to support efficiency, accuracy, communication, and client care delivery.
• Ability to work independently while collaborating effectively within a multidisciplinary team.

This is a full-time, 12-month contract position covering a maternity leave. Hours remain between 8:00am-4:30pm, Monday-Friday, and the role is primarily remote, with occasional requirements to come into the office at 150 York St. Toronto, as necessary. There is rotational on-call coverage required in this role.

This role will be reporting to Tawnya Poirier, Manager, Client Care Nurse Consultants, Year Round Care and Annual Health Assessments.

Position Pay Range:

Pay is determined based on your RN registration date.

• 1 year: $73,777.60
• 2 years: $76,003.20
• 3 years: $79,851.20
• 4 years: $83,158.40
• 5 years: $87,235.20
• 6 years: $91,249.60
• 7 years: $97,926.40
• 8+ years: $105.768.00

The above figures are based on an annual full-time salary. All amounts are in CAD.

Pay will be determined based on an analysis of the selected candidate's experience and qualifications within our nursing grid. Medcan's compensation ranges are determined by a combination of required qualifications and skills, market value, and internal equity. The above range pertains solely to the base compensation and is not inclusive of additional compensation details such as perks, benefits, and potential bonuses or incentives.
