Our unique palliative model of care spans across inpatient units, community services, supportive care clinics, and consult/liaison roles—ensuring patients and families receive holistic, compassionate care throughout the disease trajectory.
Cabrini Palliative Homecare is an integral part of the Cabrini Palliative and Supportive Care Service and provides a 24-hour service to support patients in the community.
What the role involves
We are seeking a compassionate and experienced Palliative Community Registered Nurse to provide high-quality care in the home setting.
In this role, you will:
Provide comprehensive and ongoing assessment and guidance to manage symptoms experienced by patients with a life-threatening illness in their homes
Deliver expert symptom management and end-of-life care in patients’ preferred location (where appropriate)
Support patients, families, and communities across diverse ages and cultural groups
Use evidence-based nursing knowledge and clinical skills to guide care
Practice interdependently, taking accountability for your own actions and delegating to enrolled nurses and health care workers in line with legislation and Cabrini protocols
Work autonomously while being supported within an interdisciplinary team
Ensure all care is delivered within the framework of Cabrini’s Mission, Vision, and Values
What we're looking for
Current unrestricted AHPRA registration as a Registered Nurse
Current Police Check & Victorian Working with Children Check
Current Victorian Driver’s Licence
Experience in palliative care
Excellent communication and interpersonal skills
Ability to work both independently and within a team
Desirable (not essential):
Postgraduate qualification in Palliative Care or Oncology
Previous community nursing experience
