
ChristianaCare's Weight Management Department in Wilmington, DE is seeking an experienced Case Manager - RN to deliver high-quality, safe, and evidence-based patient care.
The Case Manager serves as the primary clinical resource for patients throughout their weight management journey, providing comprehensive care coordination, patient education, clinical triage, and navigation services. Working collaboratively with physicians, advanced practice providers, dietitians, behavioral health specialists, pharmacists, and community partners, this role helps ensure patients receive timely, evidence-based, and patient-centered care. The Case Manager supports improved clinical outcomes by coordinating services, addressing barriers to care, enhancing access to resources, and promoting an exceptional patient experience while optimizing healthcare utilization across the continuum of care.
The ideal candidate will have experience in weight management, obesity medicine, bariatric services, or a related specialty, with the ability to assess patient needs, provide clinical triage, and guide patients through their care journey. Previous experience with patient navigation, care coordination, and population health management is highly preferred.
The successful candidate will possess strong organizational, communication, and problem-solving skills, with the ability to manage multiple priorities while maintaining accurate documentation and timely follow-up. Proficiency with Epic and other healthcare technology platforms is essential. This individual must be collaborative, compassionate, and patient-focused, while also being comfortable working independently in a hybrid environment.
Candidates should demonstrate the ability to build strong relationships with patients and interdisciplinary team members, identify and address barriers to care, and support patients throughout the weight management and bariatric process. A strong understanding of the patient experience and a commitment to delivering high-quality, coordinated care are key to success in this role.

Hourly Pay Range: $38.98 - $62.38
This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
