Job Title: Registered Nurse (RN) Case Manager – Home Health | Build Lasting Patient Relationships Company: Home Health Care, Inc. What You'll Do Manage a home health caseload throughout Bloomington and Eden Prairie , with on-call and weekend visits within approximately 35 miles of your home. Complete OASIS assessments and individualized plans of care. Deliver skilled nursing care and patient education. Coordinate care with physicians and interdisciplinary team members. Help patients achieve their goals while remaining safely at home. Maintain timely and accurate documentation. What We're Looking For Active Minnesota Registered Nurse (RN) license. 3+ years of RN experience (home health experience preferred). Strong clinical assessment and critical thinking skills. Excellent communication and organizational abilities. Reliable transportation for local travel.
