The Chronic Care Case Management Nurse supports Medicare patients through assessment, planning, facilitation, and advocacy to promote effective, cost efficient health outcomes. This role helps patients understand and use their health plan benefits; coordinates needed services and works to remove barriers to appropriate care. The nurse functions both independently and collaboratively within a multi specialty clinic environment and may rotate among clinics, primarily conducting non–face to face encounters with the Medicare population.

Monday - Friday, 8:00AM - 5:00PM. Some travel required.
