Mount Auburn Hospital is seeking an experienced Registered Nurse Case Manager to join our collaborative inpatient care team. In this role, you’ll play a vital part in guiding patients through safe, effective transitions of care—working closely with physicians, nurses, social work, payers, and post acute partners to ensure the right care, at the right time, in the right setting.

What You’ll Do
As a Registered Nurse Case Manager, you will serve as a clinical resource and coordinator, helping patients navigate complex health care needs while supporting excellent outcomes and efficient care delivery.
• Apply established clinical criteria to assess patient needs and identify opportunities for care in the most appropriate setting
• Partner with physicians, nursing, social work, patients, and families to develop and implement individualized post hospital plans of care
• Coordinate timely, safe discharges to home, skilled nursing facilities, rehabilitation centers, or other post acute settings
• Communicate accurate, timely clinical information to insurance providers to support admission status determinations and reimbursement
• Act as a key liaison between patients, families, hospital teams, and post acute care providers to ensure smooth transitions of care
• Support continuity of case management services by participating in weekend, holiday, and on call (beeper) rotations
• Ensure compliance with all applicable insurance, contractual, accreditation, and regulatory requirements
• Follow Mount Auburn Hospital Case Management Standards, policies, and best practices
• Complete other related responsibilities as needed to support the department and patient care operations

Pay Range:
$79,268.80 USD - $204,318.40 USD
The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
