Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Continuum Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.
Schedule: Monday - Friday, 8 am - 4:30 pm
Territory: Sonoma and Marin County
In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives The Best Care Possible — care grounded in compassion, clinical excellence, and connection.

We offer a supportive, growth-oriented environment along with a comprehensive benefits package that includes:
$64.00 - $66.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
