Job Description

The RN Home Services Case Manager utilizes clinical expertise and critical thinking skills to direct resource utilization to develop and implement a plan of care that is both cost effective and is designed to assist the patient in moving to functional independence and improve quality of life. The RN Home Services Case Manager is the primary caregiver upon admission, recertification, and discharge for the purpose of assessing progress toward stated goals. This role is responsible for the completion of the Outcome and Assessment Information Set (OASIS) and tracking progress against admission assessment. The incumbent has accountability for the coordination of caregivers and supplies necessary in the provision of patient care. This position works collaboratively with patients, patient families/significant others, healthcare providers, insurers, community resources, and other involved parties.

Home Health RN Case Manager

We're looking for a dynamic and compassionate RN Case Manager to join our Home Health team. In this vital role, you’ll lead the coordination and delivery of exceptional patient care within the home setting—serving as a central figure on our interdisciplinary team.

You’ll take ownership of the entire patient care journey, from admission through discharge, ensuring seamless communication, high-quality outcomes, and efficient resource use. Your leadership will guide planning, education, and collaboration across disciplines, while fostering strong relationships with patients, families, and payors.

Minimum Qualifications:

  • Current Registered Nurse (RN) license in state of practice. Valid compact licenses must be transferred to the state of practice within 60 days of the start of employment.

  • Bachelor of Science in nursing (BSN) from an accredited institution (degree verification required).

  • RNs must obtain their BSNs within four (4) years of hire or promotion date. If there is an existing education agreement, that agreement will take precedence.*

  • Basic Life Support Certification (BLS) for Healthcare Providers.

  • Current driver’s license, insurance, reliable transportation, and an acceptable driving record.

  • Unit specific certification as required (i.e., ACLS, PALS, etc.).

  • Demonstrated clinical nursing experience.

  • Demonstrated knowledge of home health or hospice.

  • Ability to communicate effectively both verbally and in writing.

* Bachelor of Science in Nursing (BSN) is required as of the job description's last update July 2025. Employees hired or promoted prior to this date will be held to the minimum requirements that were in place at the time of their promotion or hire.

Preferred Qualifications:

  • BSN from an accredited institution (degree will be verified)

  • Case Management Certification

  • One (1) year experience working in home health or hospice.

  • Three (3) years of clinical nursing experience.

  • Experience working with third-party payors

  • Previous experience in case management, utilization review, and/or discharge planning.

Physical Requirements

  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
  • Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
  • Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
  • May be expected to stand in a stationary position for an extended period of time.
  • For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$41.20 - $62.17
