Registered Nurse, Case Manager

The Registered Nurse serves as the case manager for assigned patients and as the teacher for patient’s caregiver(s). The RN case manager utilizes the nursing process and the six phases of the Alive CARE visit to provide safe, end of life care to the to the terminally ill patient/family unit. The primary responsibility of the RN case manager is to coordinate and provide hospice care for home care patients through the development, initiation and updating of a plan of care in collaboration with patient and family RN case managers provide direct patient care, oversee the interdisciplinary team and teach patients and families regarding the disease process, symptom management and offer support and strength during difficult times. Care delivery takes place in the patient’s home or in any other facility where the patient may reside or receive care. The RN case manager will be accountable for meeting Alive Care Standards and participating in an on-call rotation.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The Alive CARE Visit
Present a Professional Image
Represents Alive in a positive manner. Takes pride in personal appearance, conforms to Alive standards of dress code (logo wear) , car cleanliness, driving etiquette and appropriately prepared for visits. Presents self to the public in a calm, confident, competent manner
Provide Excellent Customer Service
Conducts visit in a professional, courteous manner, projecting warmth during interactions with facility staff, patient & family. Engages in purposeful conversation using appropriate body language and active listening skills
Assess, Plan, Intervene and Educate
Utilizes critical thinking skills and nursing process to holistically assess patient and family, develop and implement an interdisciplinary hospice plan of care in collaboration with facility staff, initiate interventions and evaluate effectiveness. Educates facility staff, family/caregiver(s) regarding plan of care, interventions, diagnosis and prognosis, medications, use, administration and side effects, how to provide care, use of equipment and supplies and how to contact Alive for assistance
Document at the point of care
Completes documentation of patient visit by verifying visit frequency and documenting accurate reflection of patient's condition during the patient visit; updates plan of care as needed. Ensures facility staff has documentation of care provided.
Create a lasting impression
Concludes patient visit by providing encouragement to facility staff and caregiver(s), ensuring emergency phone number (Call Center Number) and appropriate amount of supplies/medications are available. Engages in active listening and expresses gratitude before leaving home.
Complete post visit follow-up
Concludes entire encounter according to Alive CARE visit standard: follow up messages to team members, synchronize iPad, and spiritual release of the visit.
Requirements
EDUCATION and/or EXPERIENCE
Registered Nurse with a current nursing license, BSN preferred. Knowledge of or willingness to learn hospice care. Minimum one-year nursing practice or equivalent experience required. Hospice, Oncology, or Home Health and/or long-term care preferred.
CERTIFICATES, LICENSES, REGISTRATIONS
Current Tennessee RN License or compact license. Hospice certification desirable. If required to drive to carry out the duties of this position: current driver's license and automobile insurance as required by Tennessee State Law.
