
Job Title: Registered Nurse - Care Manager
Department: Greater Rochester Independent Practice Association (GRIPA)
Location: 100 Kings Highway South, Rochester, NY
Hours Per Week: 40 hours
Schedule: Full-time, Days
This position will be employed by the Greater Rochester Independent Practice Association, INC. (GRIPA). GRIPA was founded in 1996 as a unique collaboration between hospitals and physicians, for the purpose of simultaneously improving the quality and efficiency of health care. This position will not be employed by Rochester Regional Health (RRH).
The RN Care Manager will provide direct patient support both in-person and remote. They will be responsible for orchestrating care management across the continuum, coordinating care needs with physicians and care providers, developing care plans, implementing interventions, and educating patients on chronic condition management. The RN Care Manager will act as part of an interdisciplinary care team.
RESPONSIBILITIES
Providing patient directed and disease specific self-management education and psychosocial support
Monitoring and measuring patient progress and assure timely and complete documentation following any patient related contact
Work in settings across the care continuum including hospitals, provider practices and patient’s home and/or community settings
Working in collaboration with all members of the care team to assess, facilitate, plan and advocate appropriate services to meet the patient’s health requirements on an individual basis
Facilitating quality, cost-effective medical management
REQUIRED QUALIFICATIONS
BSN (preferred) or Associate’s Degree in Nursing (ASN) with a Bachelor’s degree in another discipline
Five (5) years clinical experience in care management, internal medicine, acute care or home-based experience
Excellent communication and interpersonal skills
Ability to navigate complex medical and social systems
Exceptional documentation and planning skills
A collaborative and flexible spirit
Strong computer skills (competence in Microsoft Office, particularly Word, basic Excel, Outlook, Teams, and familiarity with electronic medical records)

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
