The Aortic Center Clinic (ACC) is a multidisciplinary program comprised of physicians, advanced practice providers, and nursing staff dedicated to the comprehensive care of patients with complex aortic disease.
Patients referred to the ACC require coordinated diagnostic testing and multiple specialty consultations to ensure accurate diagnosis and optimal treatment planning.
The Nursing Program Coordinator, in close collaboration with the ACC interdisciplinary team, plays a key role in coordinating and facilitating the clinical care of patients referred for ACC services. This role ensures seamless care delivery across the continuum, supporting timely evaluations, testing, and follow-up while serving as a central point of communication for patients and providers.

The Nursing Program Coordinator works with physicians and advanced practice providers to coordinate the care for patients who are referred for and receiving care for Aortic Center Program services. The Nursing Program Coordinator provides leadership by serving as the patient’s educator, consultant, and liaison for the medical, surgical, and nursing staff. The Nursing Program Coordinator participates in the coordination of patient care activities in collaboration with the multi-disciplinary health team, identifies and evaluates current clinical practices through performance improvement, and provides education when appropriate. The coordinator will manage the ACC database and identify care provision opportunities to integrate into clinical workflows.
