The Assertive Community Treatment (ACT) program provides intensive, team-based, community-delivered services to adults with serious and persistent mental illness, with services available seven days per week.
This position is part time spent in the ACT team, in a job share with another RN, and part time spent in our RMHC outpatient nursing services. For ACT: your work is direct patient care and administrative nature and is designed to meet the needs of individuals served by the ACT program in alignment with CCBHC standards. The Registered Nurse works as part of a multidisciplinary ACT team to provide nursing services, medication management, health education, and crisis intervention in community settings. The RN collaborates closely with psychiatric providers, case managers, and other team members to ensure coordinated, recovery-oriented care. For outpatient nursing services at RMHC: it outpatient psychiatry, ARMHS (Adult Rehabilitative Mental Health Services), and Wellstone Crisis and IRTS (Intensive Residential Treatment Services). Working in ARMHS means providing mental health, physical health, medication education and other education and support services directly to patients in their homes and in our service locations. Outpatient psychiatry is a traditional clinic but also includes CCBHC clinical standards such as assuring patients are meeting their physical health needs and following up after a hospital or ED visit. At Wellstone, you are assuring the practitioners are safely providing care given they are not nurses and are checking in with patients on their physical and mental health to support them and the staff.

Range Mental Health Center (RMHC) was the first rural community mental health center in the United States and the first in Minnesota to provide services developed specifically for persons with serious and persistent mental illness. The Center was established in 1961 and was the 16th federally designated community mental health center in the nation, and the first in Minnesota.
Mental health services are provided each year to over 5,000 adults, children, and families. The service area of RMHC covers northern St. Louis County, and expands for more than 6,800 square miles.
RMHC currently employs approximately 200 individuals who provide many services, including:

RMHC also owns the Perpich Apartments, a 27-unit apartment building in Hibbing, Minn., to provide adequate housing units for individuals with severe mental illness and chronic homelessness.
