About the Job:
The Registered Nurse (Ambulatory) provides professional nursing care to patients under the supervision of providers and the Practice Manager. This role supports patient care delivery across Foundation Medical Partners practices, ensuring high-quality, coordinated, and patient-centered care in a fast-paced outpatient environment.
What You’ll Do:
Deliver nursing care across in-person visits, telephone triage, and telehealth, following professional standards and the Nurse Practice Act
Assess patient needs, develop care plans, implement interventions, and evaluate outcomes
Collaborate with physicians, advanced practice providers, and interdisciplinary teams to coordinate care
Educate patients on medications, self-management, nutrition, and overall wellness
Prepare for patient visits through pre-visit planning and preventive care review
Assist with exams, procedures, and treatments within scope of practice
Administer medications, vaccines, and perform point-of-care testing
Accurately document care in the electronic medical record
Delegate tasks appropriately to clinical and administrative staff
Respond to emergent situations and uphold safety, infection control, and HIPAA standards
Provide mentorship and clinical leadership to team members
Participate in ongoing education and competency training
Who You Are / Requirements:
Associate Degree in Nursing from an accredited program required; BSN preferred
Active Registered Nurse license in NH, MA, or compact state required
Current BLS/CPR certification required
1+ year of nursing experience and ambulatory care experience preferred
Strong knowledge of nursing practices, EMR systems, and patient care standards
Excellent communication, time management, and organizational skills
Ability to work independently and collaboratively in a fast-paced environment
Compassionate, detail-oriented, and adaptable to change
