Job Description

Park Nicollet is looking to hire a Registered Nurse to join our Advanced Care Primary Care team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Utilizes a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation and evaluation in accordance with the Nurse Practice Act, scope of practice, along with recognized professional standards and Health Partner and employer specific protocols and policy.

About Us: The Advance Care Primary Care Clinic is dedicated to supporting patients with complex chronic illnesses who are at higher risk for hospital admission. Our clinic offers a collaborative environment where patients work closely with a multidisciplinary care team, including Pharmacy, Nursing, and Social Work, to receive comprehensive wrap-around services.

We are seeking an experienced Registered Nurse (RN) to join our Advance Care Primary Care Clinic team. The ideal candidate will have a strong background in care coordination and acute care nursing, with proficiency in EPIC and a commitment to interdisciplinary communication and collaboration.

Key Responsibilities:

  • Provide comprehensive nursing care to patients with complex chronic illnesses.
  • Coordinate patient care with the multidisciplinary team, including Pharmacy, Nursing, and Population Health.
  • Utilize EPIC to document patient information and manage care plans.
  • Follow established protocols to ensure high-quality patient care.
  • Engage in interdisciplinary communication and collaboration to optimize patient outcomes.
  • Educate patients and their families about managing chronic illnesses and available resources.

Work Schedule:

  • Monday-Friday

  • 8:00am-5:00pm

  • 40 hours per week/1.0 FTE

Required Qualifications:

  • Maintain a current Minnesota registered nurse’s licensure.
  • WI licensure required where applicable.
  • Current BLS provider status in accordance with American Heart Association (AHA) guidelines
  • No online only courses accepted.
  • 1-2 years of experience in ambulatory nursing.
  • 1-2 years of experience in acute care nursing.
  • Proficiency in EPIC.
  • Strong skills in interdisciplinary communication and collaboration.
  • Ability to work effectively in a team-oriented environment.

Preferred Qualifications:

  • Bachelor’s Degree in Nursing
