Job Description

As an RN Case Manager, you'll be the driving force behind a patient's journey through the healthcare system, ensuring they receive the right care, at the right time, in the right place. Working closely with physicians, nurses, therapists, social workers, patients, and families, you'll help remove barriers, coordinate services, and create safe, successful transitions throughout the continuum of care.

This role is ideal for nurses who love solving problems, building relationships, and thinking beyond the bedside. Every day brings opportunities to advocate for patients, navigate complex situations, and make a meaningful impact on outcomes, quality of care, and the patient experience.

Requirements

  • Active Indiana/Kentucky RN license or valid compact RN license
