The Registered Nurse supports the delivery of evidence-based chronic disease programs by providing clinical oversight, participant education, and health monitoring. This role works collaboratively with program staff to ensure safe, effective, and engaging programming for individuals managing conditions such as diabetes, hypertension, heart disease, and arthritis.
· Conduct pre-program health screenings and review participant medical history as appropriate
· Monitor participant health status during programs and respond to any medical concerns or incidents
· Ensure all programs follow established health, safety, and risk management guidelines
· Maintain accurate and confidential health documentation in compliance with HIPAA standards
· Provide basic health education related to chronic disease prevention and management
· Reinforce healthy lifestyle practices including nutrition, physical activity, medication adherence, and self-monitoring
· Answer participant questions within scope of practice and refer to appropriate providers when needed
· Support behavior change by encouraging participant goal setting and accountability
· Partner with program coordinators, wellness staff, and instructors to support program delivery
· Offer clinical insight to help adapt activities for participants with varying health conditions
· Participate in program planning, evaluation, and quality improvement efforts
· Respond to medical emergencies and activate emergency protocols as needed
· Ensure readiness of emergency equipment (e.g., AED, first aid supplies) during program hours
· Current Registered Nurse (RN) license in the state of New Jersey
· CPR/AED and First Aid certification (required)
· Minimum 1–2 years of clinical experience, preferably in community health, wellness, or chronic disease management
· Strong communication and interpersonal skills
· Ability to work independently in a part-time, flexible schedule
· Experience with community-based programs (e.g., YMCA or similar wellness settings)
· Familiarity with evidence-based programs such as Diabetes Prevention Program (DPP), or Livestrong at the YMCA
· Experience working with diverse adult populations, including older adults
