Job Description

Compensation Range: $16/hr - $19/hr commensurate with experience

Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.

Aplicar estrategias específicas para admitir participantes y proporcionar cuidado o servicios relacionados con salud primaria, HCV, HIV, trastornos por consumo de sustancias (SUD) y/o los trastornos concurrentes (COD) a las comunidades con mayor prevalencia de SUD/COD, entre otros.

Responsibilities:

  • Brindar servicios tales como: evaluaciones, cernimientos, cuidado o referidos a los participantes que sean admitidos.
  • Evaluar las necesidades de los participantes y hacer recomendaciones al equipo clínico.
  • Participar del desarrollo de los planes de tratamiento y promover el cumplimiento de estos, si fuese necesario.
  • Educar a los participantes del programa sobre MAT, VIH, Hepatitis C, Prevención de sobredosis u otros temas relacionados.
  • Referir participantes a otros proveedores internos o externos y ofrecer seguimiento.
  • Participar de actividades de alcance comunitario para identificación, búsqueda y reclutamiento de participantes en colaboración con el equipo. Completar documentos alineados a estas intervenciones (Hoja de contacto, SUPRT, otros)
  • Participar en las reuniones de discusión de casos, según sea necesario o solicitado.
  • Tomar signos vitales a participantes y asistir al medico en lo que sea requerido.
  • Ofrecer servicios de enfermería y seguimiento en distintos escenarios tales como: Espacio Clínico, Comunidad y Unidad Móvil.
  • Manejo de toda información confidencial, según las reglamentaciones federales y estatales.
  • Brindar servicios basados en las guías o utilizando las mejores prácticas e intervenciones basadas en evidencia científica.
  • Ofrecer servicios de cuidado de heridas/úlceras segun sea necesario.
  • Completar documentación necesaria en expediente de participantes o registros de enfermería (Desfibrilador, Botiquín, otros) dentro del tiempo estipulado por la organización. Rendir informes cuando sea requerido.
  • Promover relaciones colaborativas con otros proveedores basadas en las necesidades de los participantes y la organización.
  • Participar del desarrollo de protocolos clínicos de ser requerido
  • Asistir en el transporte o acompañamiento de participantes (o del equipo) según sea requerido.

Requirements:

  • Evidencia de grado de enfermería
  • Conocimiento sobre la problemática de uso de sustancias, VIH/SIDA, Hepatitis C y ETS en Puerto Rico
  • Poseer una alta sensibilidad y dedicación hacia la población de usuari@s de drogas y otras poblaciones marginalizadas
  • Conocimiento básico sobre implementación de modelo de salud pública
  • Conocimiento sobre ulceras por punción
  • Licencia de conducir vigente y dispuesto/a a conducir vehículos oficiales
  • Debe ser enfermero/a registrado/a, con una licencia activa
