Your Opportunity

This is a permanent full-time opportunity, working Monday to Friday from 9:00am to 5:00pm and based on-site at Castle Hill Village.

As a Registered Nurse, you will deliver clinical care and nursing support to clients living within our Seniors Communities. You will conduct clinical assessments,monitorclient health, contribute to careplanningand provide nursing interventions that support clients to remain safe and well in their chosen environment.

You will play akey role in ensuring clinical assessments, care plans and clinical documentation are current,comprehensiveand regularly reviewed to support ongoing clinical compliance ,bestpracticeand quality resident outcomes. You will alsomonitorhigh-risk residents, coordinate follow-up care following hospital discharge, and respond proactively to changes in residents' health and wellbeing.

Working autonomously and as part of a multidisciplinary team, you will ensure clinical needs areidentified,documentedand actioned in line with aged care standards, regulatoryrequirementsand Anglicare's commitment to person-centred care.

Clinical Care & Assessment

• Deliver routine and complex nursing care, including wound care,injectionsand clinical monitoring

• Conduct comprehensive health and clinical assessments andidentifychanges in resident condition

• Casemanagesresidents' clinical care to ensure ongoing health,wellbeingand quality outcomes

• Monitor residents identified ashigh riskand coordinateappropriate interventions

• Review residents following hospital discharge and coordinate ongoing care requirements

• Escalate clinical concerns appropriately and liaise with GPs,specialistsand allied health providers

• Promote positive health outcomes through early intervention and preventative care practices

Care Planning & Clinical Documentation

• Develop,reviewandmaintaincomprehensive assessments, careplansand clinical documentation

• Ensure care plans reflect residents' physical, emotional,socialand spiritual needs

• Complete documentation accurately and within requiredtimeframes

• Ensure compliance with organisational policies, clinicalguidelinesand legislative requirements

• Support ongoing clinical governance and quality assurance requirements

Resident & Family Engagement

• Build trusted relationships with residents,familiesand representatives

• Support residents and families through case conferences and clinical discussions

• Provide clear communicationregardingresident health, careplanningand clinical outcomes

• Ensure residents are treated with empathy,dignityand respect in all interactions

Teamwork & Knowledge Sharing

• Share clinical knowledge andexpertisewith colleagues and care teams

• Foster collaborative relationships with clients,familiesand healthcare professionals

• Support a culture of continuous improvement,learningand professional development

Safety & Wellbeing

• Promote andmaintainsafe clinical practice

• Participate in workplace health and safety activities

• Report incidents,hazardsand risksin accordance withorganisational requirements

What You'll Bring

You are a compassionate and capable Registered Nurse who enjoys building meaningful relationships and making a genuine difference in the lives of older people.

You will also need:

• CurrentAHPRA Registration as a Registered Nurseis essential

• Minimum3 years' nursing experience

• Experience in aged care, communitynursingor a related healthcare setting (preferred)

• Strong clinical assessment and care planning skills

• Excellent written and verbal communication skills

• Ability to work autonomously and manage competing priorities

• Sound computer skills and confidence using clinical documentation systems

• Current Driver's Licence and reliable vehicle
