Position Type: Full Time
Salary Range: $55.21 - $64.50 Hourly

Job Category: Health CareThe Registered Nurse, under general supervision, is responsible for assisting Health Center Physicians in delivering comprehensive patient care. The RN triages and coordinates with the healthcare team to ensure quality services. This role involves conducting health education, providing direct nursing services, and promoting the overall well-being of patients and clients within a primary care setting.

Duties and responsibilities
- Provides direct patient care within the scope of nursing practice based on patient needs.
- Assist with developing individualized care plans and care coordination in collaboration with Enhanced Care Management.
- Triages incoming patient calls and walk-ins, providing clinical advice as appropriate.
- Reviews lab results, imaging, and other diagnostic tests; collaborates with providers to address abnormal findings.
- Assists in coordinating referrals to specialty care providers.
- Provides patient education and advocates for patient needs.
- Collaborates with integrated care teams including physicians, behavioral health counselors, and other healthcare staff.
- Supports quality improvement initiatives to enhance patient outcomes.
- Maintains accurate medical charts, records, and prepares reports as necessary.
- Adheres to ACCI’s policies, procedures, and Standing Orders to ensure compliance and quality care.
- Facilitates patient and employee screenings in accordance with HIPAA regulations and agency protocols.
- Performs monthly checks of Emergency Carts and Oxygen Tanks to ensure readiness.
- Provides culturally respectful care to a diverse patient population.
- Models and promotes a collaborative, team-based approach to patient care.
- Completes other duties and related projects as assigned.

Qualifications
-  Ability to perform the duties described above
Education and Experience
- Bachelor of Science in Nursing (BSN) degree required
- Active Registered Nurse (RN) license in the State of California required
- 2 years of nursing experience, preferably in primary care or outpatient settings
- Current BLS (Basic Life Support) certification required
- ACLS (Advanced Cardiovascular Life Support) and PALS (Pediatric Advanced Life Support) certifications preferred
Knowledge, Skills, & Abilities:
- Knowledge of nursing principles, clinical practices, and regulatory requirements specific to primary care and outpatient settings.
- Ability to perform comprehensive health assessments, screenings, and routine physicals.
- Proficient in-patient education on chronic disease management, wellness, and prevention strategies.
- Proficient in interpreting diagnostic tests common in primary care (labs, EKGs, screenings).
- Proficient in Electronic Health Records (EHR) systems with precise documentation tailored to outpatient care.
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint) for creating patient reports, tracking health data, and preparing educational materials.
- Ability to apply critical thinking and adapt in dynamic outpatient environments.
- Ability to coordinate care and communicate effectively with patients, families, and interdisciplinary teams.
- Ability to respond promptly to urgent care situations within the primary care scope.
- Knowledge of ethical standards, patient confidentiality, and compassionate, continuous care practices.
- Verbal and written fluency in English required; Bilingual skills in a second language (particularly Spanish, Mandarin, Vietnamese) desired but not required.

- Ability to establish and maintain effective work relationships as part of a multi-disciplinary team
Working conditions
Work is often conducted in an office environment inside a standard clinic setting. Basic safety precautions and
use of protective clothing or gear is required. Occasional evening and weekend work is required.
