Job Description

This position is for a Registered Nurse who wants to continue using their clinical knowledge without providing direct or physical patient care.

The RN in this role completes in-home visits by talking with clients and families, review health history, and develop clear, practical care plans for non-medical in-home caregivers to follow. The work is focused on assessment, communication, and care planning—not clinical tasks.

This role is centered on conversation, observation, documentation, and relationship-based support.

What you will be doing

  • Travel to clients’ homes to complete scheduled, non-clinical nursing assessments

  • Conduct verbal and observational assessments with clients and family members

  • Review medical history, diagnoses, medications, functional abilities, and daily routines

  • Identify risks, support needs, and appropriate non-medical services

  • Develop and update individualized care plans for in-home caregivers to follow

  • Serve as a clinical resource to the care team regarding the client’s plan of care

  • Maintain accurate and timely documentation of assessments and care plans

The focus of each visit is understanding the client’s health background, functional needs, and personal goals in order to build a realistic and supportive care plan.

  • Want to continue using nursing judgment without bedside responsibilities

Qualifications

  • Active Registered Nurse (RN) license

  • Ability to complete thorough verbal and observational assessments

  • Strong documentation and care-planning skills

  • Comfortable working independently in the community

  • Professional, compassionate communication style with clients, families, and caregivers

Work environment

This role includes traveling to clients’ homes for scheduled assessment visits and collaborating with the office team to support care delivery. There is no direct caregiving.
