Job Description

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Registered Nurse (RN) – Adult Day Program
Stoughton, MA
$38/hour
Day Shift: Monday–Friday, 7:00 AM – 3:30 PM
The MENTOR Network is a mission-based organization dedicated to providing high-quality services to those we serve. We are currently seeking a compassionate and dependable Registered Nurse (RN) to join our Adult Day Program team in Stoughton, MA. This is a daytime position with no overnight shifts, offering a consistent schedule and the opportunity to make a meaningful impact in the lives of adults and seniors in our care.

The Registered Nurse is responsible for providing nursing services, training, and support to address the medical or behavioral needs of individuals in our program. The RN will work closely with interdisciplinary teams to support participant health, wellness, and quality of care.
Essential Responsibilities:
• Conduct medical assessments and assist with care planning
• Participate in the development and implementation of individualized service plans
• Review and maintain nursing documentation, medication administration records, and physician orders
• Monitor participant health and communicate concerns to the appropriate team members
• Administer medications according to established guidelines
• Promote health, wellness, and preventative care practices
• Provide training and support to staff regarding medical needs and protocols
• Coordinate with physicians, specialists, families, and care teams
• Participate in meetings and provide nursing consultation to the interdisciplinary team
Qualifications:
• Graduate of an accredited RN program
• Current Massachusetts RN license required
• Current CPR/First Aid certification
• One year of experience working with medically complex individuals preferred
• Strong communication and organizational skills
• Bilingual in Cape Verdean Creole preferred
