QualificationsOn An Average Day You Will:

Make a true difference—one patient, one family, one compassionate moment at a time.
- Develop and implement individualized care plans based on current and prior medical history
- Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
- Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
- Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
- Administer physician-ordered medications and treatments with skill and empathy
- Educate patients and their families on care techniques and symptom management
- Track and communicate changes in patient condition, ensuring timely and effective responses
- Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery
- And contribute to other supportive duties as part of a compassionate, mission-driven team
- And additional duties as assigned
What You'll Bring to The Team:
- A heart for hospice care and a commitment to exceptional service
- A valid Registered Nurse license in good standing in the state you will work in
- A valid driver’s license and reliable transportation
- CPR certification (current)
- Strong observational and communication skills—both written and verbal
- A flexible, team-oriented attitude and willingness to support additional duties as needed
