What You’ll Do
• Provide skilled nursing care in patients’ homes
• Complete assessments and develop individualized care plans
• Educate patients and families on disease management and care needs
• Coordinate care with physicians, caregivers, and interdisciplinary teams
• Monitor patient progress and adjust care plans as needed
• Build meaningful relationships while improving patient outcomes
What You’ll Need
• Active RN license in the state applying for
• Minimum of 1 year nursing experience
• Strong clinical assessment and communication skills
• Reliable transportation, valid driver’s license, and auto insurance
• Ability to travel throughout assigned territory
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
Preferred Qualifications
• Home health or hospice experience
• Experience with HomeCare HomeBase
