Roof Above is NC’s most comprehensive homeless service provider.  Our mission is to unite our community to end homelessness, one life at a time.  Focusing on individuals experiencing homelessness, Roof Above offers everything from street outreach to shelter to Rapid Rehousing to Permanent Supportive Housing. Roof Above’s services and programs reach over 1,300 individuals every day across nine campuses in Charlotte. Roof Above was born through the merger of Urban Ministry Center and Men’s Shelter of Charlotte in 2019.
POSITION SUMMARY
The program nurse’s primary responsibility is to work as a part of a multi-disciplinary team to address physical and behavioral health needs for clients.  The nurse collaborates with the programming staff to assess needs to better ensure safe and stable housing. The nurse provides education around medications, medication management, diagnosis and completes referrals to other community agencies.
ESSENTIAL FUNCTIONS
Provide tenants with guidance and support around medical and behavioral health-related issues in a residential, non-clinical environment where care is delivered in residents’ homes, single-site offices, and common spaces rather than a structured medical setting.
Reinforces education and counseling to clients regarding identified health needs, health care activities, chronic disease management, preventive care, medication effects/side effects, and contraindications.
Perform various administrative and clinical support tasks, including documentation, coordinating appointments, obtaining vital signs, simple wound care, and supporting tenants in understanding how to self-administer their own pre-poured or prepackaged medications. This support does not include administering prescription medication.
Exercise sound clinical judgment in situations where standard medical protocols may need to be adapted to the PSH environment while remaining within the scope of practice and agency policy.
Work closely with non-clinical staff and program staff (case managers, peer support specialists, property management) to align health interventions with housing goals.
Utilize trauma-informed, culturally responsive, and harm-reduction approaches when working with residents.
Build trust-based, therapeutic relationships with residents who may have experienced trauma, system involvement, and barriers to traditional healthcare.
Respect residents’ rights to self-determination, including the right to decline services or make choices that differ from medical recommendations.
Utilize creativity and flexibility to meet residents where they are, recognizing that engagement, compliance, and health outcomes may not follow traditional clinical timelines.
Ensures confidentiality regarding sensitive material, including protected health information.
Coordinate care with primary care providers, behavioral health providers, hospitals, pharmacies, and other community health partners.
QualificationsEDUCATION, EXPERIENCE AND SKILLS REQUIRED
A Bachelor’s degree
Must be a graduate from an accredited school of nursing.
Current licensure in the State of North Carolina as a Licensed Practical Nurse or a Registered Nurse.
Experience working with chronically homeless individuals strongly preferred.
Experience working with individuals with a history of mental illness and substance use preferred.
Knowledge of, and commitment to, harm reduction and Housing First principles.
Strong written and oral communication skills.
Computer proficiency and familiarity with electronic documentation.
Superb collaborative skills and demonstrated success working in a high-needs environment.
Contribute to a program culture that values flexibility, creativity, and continuous learning.
Become certified in Homeless Management and Information Services database to enter services and view records, as needed. (Certification will be provided).
